If you didn't understand, I won't blame you. I will just explain right now if that makes it easier for you readers. Basically, when Cho was reborn, he has the natural ability to be unnoticed under any circumstance due to [redacted]. This is the reason nothing noticed him. Every time he went to "sleep," he was also absorbing all the chakra and nature energy around him. The reason he was so malnourished was because he used up all of the natural energy around him, and therefore needed to absorb in a different spot.
