It is only Vikram who is deep into an identity apocalypse, everyone else is still the same. Avi, Kane, and the Parasites are gone for good in the last chapters. Vikram is back to his unruly and uncaring self and doesn't give two hoots about other people's lives. He is a master manipulator and will continue to make use of others' emotions for his own personal gain as he himself is devoid of any EQ (emotional quotient).

However, worry not, for Karma's revenge arc alongside the appearance of hidden experts and closing-in of Sight, all kicks off at the Central District and we are already inside the den of experts (with one of them already appearing). I just hope you like reading about massacres ;p
I apologize that you felt that way, but let me clarify that Vikram doesn't have Avi's emotions. It would be a major spoiler if I tell you here, but in Chapters 15-20, you would know why he is acting the way he has been. Also, Yes! Vikram has not really been RUTHLESS per se. However, that is only because it has only been less than a week since he transmigrated and he basically has no strength or means of survival. His diabolical plans will only show up in volume 2. Thanks for reading, I hope you will be patient till Volume 2 (which is just around the corner).

No, as there are no tags for either, so there will not be any harem or romance. The world shall be a dystopia forever (at least till Volume 5) and Vikram shall remain a conflicted person trying to figure out what he should be doing with his novel knowledge.
