If you kill the beggar, you get an Oedon Writhe Caryll Rune. It heavily implies it's Oedon the formless doing all the manipulation, getting women to the chapel to impregnate them. The same can be seen for Fake Iosefka, who only appears after we defeat Gascoigne, which also implies she was hiding in the chapel and ran to Iosefka's clinic after Gascoigne is down. After the Bloodmoon, if you kill fake Iosefka, you also get Oedon's Writhe.
