The reason is because those honorifics do not have good translations in English and it is standard to use them in Japanese. The only translation would be to call him Mr. Romero all the time. You aren’t used to it because it’s rare to see Japanese in foreign settings where the language of the media is not Japanese. Anime dubs typically cut out honorifics when translating because it confuses the very young English audience. So there you go
