Considering the size of each piece torn from the whole, there should be at least five locations: 4 limbs, and maybe the head + torso. At any rate, it seems we've gotten a glimpse at how powerful the cursed hospital really is. I wouldn't be surprised if the cursed hospital has more than one demon god or maybe even an existence stronger than the average demon god.

Analysing, I think the patient that attacked the School of the Afterlife should be among the patient 0001 to 0005, maybe a little less powerful than the ghost fetus hence 0005.

I am glad the shadow/trash/0004 patient did not kill the little girl he was friends with.

Who is patient 0003? What have they been up to?
